What Happens During a Panic Attack, and What Actually Helps in the Moment

What's Physically Happening

A panic attack is your body's fight-or-flight response firing at full intensity without an actual physical threat present -- a rapid heartbeat, shortness of breath, chest tightness, dizziness and a sense of doom are all the nervous system doing exactly what it's designed to do, just at the wrong moment.

Why It Can Feel Like a Heart Attack

Many of the physical sensations of a panic attack (chest pain, racing heart, shortness of breath) genuinely overlap with cardiac symptoms, which is part of why panic attacks are so frightening and why many people's first panic attack results in an ER visit -- a reasonable and safe response if you're not sure what's happening, particularly the first time.

What Actually Helps While It's Happening

Slowing your exhale (the physiological sigh or 4-7-8 breathing both work here), naming what you can see and hear around you (grounding), and reminding yourself that panic attacks peak and then pass -- typically within 10-20 minutes -- can shorten the intensity, even though it can feel endless in the moment.

What Doesn't Help

Fighting the sensations, trying to "calm down" through sheer willpower, or catastrophizing about what the sensations mean tends to intensify a panic attack rather than shorten it -- accepting that it's happening and riding it out with a grounding or breathing technique works better than resisting it.

When to Take It Further

If panic attacks are recurring, unpredictable, or starting to shape your behavior (avoiding places or situations out of fear of having one), that's a pattern worth discussing with a doctor or therapist -- panic disorder is a treatable, well-understood condition, not something you have to just manage alone indefinitely.
